The Family Life Office offers several options to register for events.
  • Register onlineThe online registration system allows you to register for the event, and reserve your spot. You can pay online with a major credit card and get an instant confirmation of payment and registration.
  • Download a registration form. Download a registration form to complete and return by mail. You can include a credit card number for payment of registration fees, or send a check payable to Family Life Office. You will receive a confirmation of payment and registration within 14 days. The registration form is in PDF format. To access it you must have Adobe's Acrobat Reader installed. You can download Acrobat Reader for free.
  • Request a registration form. Call 1-815-965-5011, use the online contact form, or write to us at 7708 E. McGregor Rd. Rockford IL 61102 to request a registration form be sent to you in the mail. You can complete the registration form and return it by mail or fax. You can include a credit card number for payment of registration fees, or send a check payable to the Family Life Office. You will receive a confirmation of payment and registration within 14 days.

Important things to Remember:

  • All events are finalized based on availabilty.
  • Prices are subject to change without notice.

Refund or Cancellation Policy: A full refund of registration fees will be made if written cancellation notice is received before your event.  Some events require a prior notification of atleast one or more weeks.  Requests for cancellations and refunds must be in writing.  Deposits are non refundable unless otherwise stated.
